All I could find was a toggle for Heart Rate Monitoring in the Bluetooth stack of iOS 19. I don't believe that is what starts streaming the heartrate. There must be something else that actually requests the data. If someone has the following, then I can document it and add to Android

The latest Pro AirPods have the ability to automatically check your heartrate similar to a fitness tracker. Would be nice to support it some way or another, whether an in-app database or connecting to the Health APIs of the device.
