fix(mcp): send mirrored headers from the stdio-to-HTTP bridge - #2004
When pipelock mcp proxy --upstream URL talks protocol revision 2026-07-28 to an HTTP server, it now sends Mcp-Method, Mcp-Name and the Mcp-Param-* mirrors that x-mcp-header annotations ask for. It builds them from the arguments it forwards, after any redaction, with the same encoding the HTTP listener checks. If it hasn't seen a tool's header contract yet it fetches the tool list privately first, and that lookup can't change the bridge's session. A call whose contract is invalid or can't be established, or a tools/call, resources/read or prompts/get sent without an id, is refused locally instead of going out without its headers. Once it negotiates 2026-07-28 it keeps that revision for the life of the process and refuses a second initialize. Earlier protocol revisions get no new headers.
A held call on this bridge can now be released by a scanned tool list (allow_on.tool_inventory_baseline), which never fired on this transport before. With tool scanning off, the bridge still reads tool lists for header contracts and leaves out tools whose contracts are invalid, but that read never releases a held call.
Summary by CodeRabbit
New Features
For protocol revision 2026-07-28 and later, the HTTP bridge sends routing and parameter headers based on the final forwarded arguments, omitting headers for absent or null values.
The bridge can privately discover unknown tool contracts while preserving session context and applying shared limits. Calls are refused when discovery fails or arguments cannot be safely mirrored; unchecked fallback and automatic replay are not performed.
After negotiating a modern protocol revision, the bridge retains it and rejects a second initialization. Relevant requests without an ID are refused.
Documentation
Updated the configuration reference with the bridge’s header and contract-discovery behavior.
A best-effort allow is a single-phase decision receipt, not an intent.
Retaining its identity for every current-protocol request made each
forwarded call, on the bridge and the HTTP listener, close it with an
outcome receipt, which completeness analysis reports as a broken chain.
Only a required allow is closed by an outcome again.
A deferred call refused at release is closed by its resolution receipt,
as the live upstream gate does; its hold wrote a defer receipt, so an
extra outcome under the defer ID read as an outcome without an intent.
When the operator configured no tool scanning, the bridge still collects
tools/list structure to synthesize header mirrors. Its clean result only
means nothing was inspected, so it must not be the confirmed inventory
that allow_on.tool_inventory_baseline waits for. Scanned inventories keep
releasing such holds; structural ones leave them to another signal or
the hold timeout.
A legacy per-request protocol version no longer downgrades a session negotiated at 2026-07-28, which dropped the request's routing and parameter headers. Tabs in mirrored values are Base64 encoded, as Value Encoding requires for control characters.
A modern tools/call, resources/read or prompts/get sent without an id
skipped contract acquisition and left the bridge with no Mcp-Method,
Mcp-Name or parameter mirrors. MCP defines these methods only as
requests, so the bridge now drops the notification form locally and
closes its intent receipt with a blocked outcome. Other notifications
and legacy sessions are unchanged.
The transport clears its negotiated version on a new handshake and when the session ends, and an empty version read as legacy, so later calls could leave without request metadata. The bridge now keeps the first modern version it negotiates for its lifetime.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
The HTTP bridge now synthesizes protocol-aware routing and parameter headers from tool contracts, discovers unknown contracts through private upstream requests, and checks contracts before forwarding calls. It also handles protocol-version retention, inventory-based hold resolution, and receipt outcomes for refused calls.

Merge Risk: Low · previous review aa767
If the session is cancelled while a tools/list request waits for the inventory lock, the bridge can exit without deleting the upstream session or closing the pending receipt. Under required receipts this can show up as incomplete evidence. The fix is small and local. Malformed protocol versions are now refused.

Validate the request version before forwarding.
mcpBridgeHeaders.version compares the raw request version lexically. A value such as 2026-99-99 can pass the modern gate, reach prepare, and be sent as Mcp-Protocol-Version. The HTTP listener only accepts canonical YYYY-MM-DD values and returns 400 Bad Request.
Reject invalid modern versions at this gate before header synthesis.

Stop the tools/list lock-error path from returning before cleanup.
When contractMu.Lock(ctx) fails for a tools/list request, return lockErr exits the forwarding loop directly. The tracked request outcome for that call is then never emitted. Cleanup also does not run: the session is not deleted, and emitPendingIncompleteOutcomes is skipped. Set the error and break out of the loop instead, so the normal shutdown path settles pending outcomes.

Record a terminal error outcome when a released deferred request fails to send or scan.
After release, a deferred request that fails in SendMessageWithOptions or returns a non-timeout ForwardScanned error keeps its tracked request entry. The entry stays pending until shutdown. The shutdown fallback then emits it as incomplete, so the request receipt does not record the send or scan failure when it happens. The timeout branch in the same callback emits response_timeout, and blockScanError emits response_scan_error. Apply the same pattern to these two branches.
