fix(gateway): avoid duplicated Responses history (salvage #18995)

Salvage of #18995 by @thelumiereguy onto current main.

Summary

Chained /v1/responses requests via previous_response_id stored the prior history twice per turn and replayed old tool artifacts in non-streaming output. This PR stores the agent transcript once and scopes output items to the current turn.

Root cause: AIAgent.run_conversation() returns result["messages"] containing the full internal transcript (prior history + current user + new assistant/tool messages). The API server treated it as a turn-local suffix and did conversation_history + user_message + result["messages"], duplicating everything. Long chains grew roughly exponentially.

Changes

• gateway/platforms/api_server.py: new _build_response_conversation_history() + _response_messages_turn_start_index() helpers detect transcript-shaped vs suffix-shaped result["messages"]. Used in both streaming (L1891) and non-streaming (L2195) persistence paths.
• gateway/platforms/api_server.py: _extract_output_items() now takes start_index so non-streaming response output doesn't replay prior-turn function_call/function_call_output items.
• tests/gateway/test_api_server.py: three regression tests (stored-once non-streaming, stored-once streaming, output-current-turn-only).
• scripts/release.py: AUTHOR_MAP entries for @thelumiereguy and @counterposition.

No run_agent.py contract change — run_conversation() still returns the canonical full transcript used by CLI, TUI, ACP, gateway, and batch paths.

Validation

scripts/run_tests.sh tests/gateway/test_api_server.py -q → 138 passed.
