Author the Phase R CHANGELOG entry and ADR-028 from CHANGES.draft.md, then sweep every doc for the category-home dashboard, hash routing, the augmented /api/categories CategoryStat payload, the ?category= feed/stream lens, the hub Subscribe-time fan-out, and the new test inventory (239, +hub_test.go). Update the Phase O/Q residual notes now closed (or kept unscoped by design) for the dashboard, refresh REVIEW/ROADMAP to note only Phase S remains, and delete the consumed CHANGES.draft.md.
