This PR adds documentation to clarify the two methods of installing MCP servers in VS Code and explains their differences:

What changed

Added new guide that explains:
- Command-line vs Marketplace installation methods
- Differences in generated mcp.json entries with examples
- When to use each approach
- How to pin versions and verify running version
- Troubleshooting tips
Added link to the new guide in mcp-builder/SKILL.md

Why

Users were confused about:

Why there are two ways to install MCP servers (code --add-mcp vs marketplace)
Why they produce different mcp.json entries
Which method is preferred
How to pin/check versions

This documentation helps users understand the tradeoffs and choose the right approach for their needs.

Testing
