Identifiers cleanup

#31 cleaned up the API and provided a forward-looking interface based on identifiers.org prefixes. These changes provide a façade over the actual data in the database. (e.g., "refseq" in the API is translated to/from "NCBI" in the database). This issue completes the transition by matching the database with the API as much as possible.

Tasks:

• Re-implement support for INSDC identifiers #90: Reimplement support for INSDC identifiers
• Drop VMC namespace
• Reconsider how to handle GRCh use of lower case and ambiguity characters
