What this PR does / why we need it:

Clarify that the value of the "operatortoken" is a token created by the Operator, usually by the operator's Entitlement Server.
This value may an TS.43 operator token as defined in TS.43 v12 section 14 "Device App Authentication" or a Temporary Token also defined in GSMS TS.43.

This change must be consistent throughout all ICM documents. CAMARA-API-access-and-user-consent.md mentions operator tokens in multiple places. This document also needs to be reviewed. However, CAMARA-ICM-examples.md looks fine as it is, as it neither mentions Operator Tokens nor contains examples of them.

For the login_hint format-part we decided to leave it as-is for now.

I reviewed CAMARA-API-access-and-user-consent.md and all places where "operatortoken" state that the value is a "TS.43 token".

So, I think, CAMARA-API-access-and-user-consent.md does not need to be part of this PR.
