inconsistency in library type requirements

The spec is inconsistent in how the requirements of library types are referenced.
We have:
    \* "requirements of a mutable random access iterator" vs.
    \* "requirements of a Random Access Iterator" vs.
    \* "requirements of a RandomAccessIterator"
