`/keys/query` no longer populates `failures` property in response

The response to /keys/query has a failures property which is supposed to list remote homeservers that could not be queried.

It no longer seems to be populated, leading to high CPU usage by clients (eg element-hq/element-web#27867)

Suspect it was introduced by #17273
