Docs: Some instructions can fail at max call depth

1. CREATE/CREATE2 will fail at maxCallDepth - 2 in some cases, and maxCallDepth - 1 when resurrecting.
2. EXTCODE* will fail at maxCallDepth.
3. SELFDESTRUCT will fail at maxCallDepth.

Still needs to be documented, but with low priority because the 1/64th rule prevents us from reaching the max call depth (usually).
