Merge into vscode-go

Issues that should be resolved and tasks completed prior to merging upstream:

• When I create a file and add a test, it doesn't get registered.
• When there's a build error, clicking on the message should take me to the location.
• Once I resolve a build error and successfully execute tests, the error is not cleared from the UI.
