more orchestration tweaks

add these to the right place in the README, with descriptions and in a sensible order

• make bmkg-monitor
• make bmkg-monitor-log
• make inasafe-worker
• make up

make deploy seems to just generate a deploy script - is this correct and is it still necessary? or superseded by make up?

Also, there are many changes after this issue, some part of the README.md doesn't relevant anymore.
I need to find some time to update this.
