feat: Create a template for maintainer onboarding

There should be a template which a person can use to open a ticket to become a maintainer. On that ticket we vote according to our governance docs.

We need to have this, to make sure we are transparent and record our process of how we accept (or even remove) maintainers.

votin mechanism is working see #3842, we just need to define the rules (how many 👍 required in %) also define who has voting rights, and what to do when ppl are on holiday or if we are ok waiting the 4 weeks
