Create @observablehq/create npm init template

I sketched out a generator in this repository: https://github.com/observablehq/create. Hopefully it can be a good reference or starting point, but I have no particular attachment to its content.

currently the following:

yarn add @observablehq/cli

fails with.

error Couldn't find package "@observablehq/runtime@^5.9.4" required by "@observablehq/cli" on the "npm" registry.

i think the cause that with the .npmrc config:

@observablehq:registry=https://npm.pkg.github.com

and no @observablehq/runtime module asset in github, install can't find the public runtime asset .

i talked with @cinxmo has a workaround for the direct use case, which works. however to test the create functionality the workaround isn't ideal.

can we add a runtime package to github in the same way we have the cli package?
