Replaces the current bug report form with the new GitHub Issue Forms Additionally, adds performance issue and installation issue form that I've seen common in our tracker but currently does not have a good way to report.

Forms can be tested here: https://github.com/mroeschke/issue-form-test/issues/new/choose
