Reimplement kernel / environment detection #234
Description
web/ dir isn't part of build (class) path, so SymfonyIndexingVisitor never scan app.php and app_dev.php files.
- Environments might be easy readed from app/config_%env%
- Kernel name from app/console
- Kernel instance locations from web/
