Individual thumbnails per item in the text

Right now, we only see the "thumbnail" for the first source document, and that only on the cover of the book.

We should allow a designated thumbnail image for each source, and there should be the option of having a page displayed with that thumbnail.

We should also have the option of having the cover with each thumbnail displayed in a matrix.
