This removes the date checks added in #1629 so that an email address is always required. The integration tests will start failing after 2019-03-01 00:00:00 UTC; this is the cure.

Then manually remove or edit the email record if we're specifically
testing a user without a verified email address.
Remove all code and tests having to do with the warnings we issued and
the date checking around the cutover date.
