Jump to definition doesn't generate a link for macro calls

On [this page](https://doc.rust-lang.org/nightly/nightly-rustc/src/rustc_errors/diagnostic_builder.rs.html), the macros calls (`forward!`, `concat!`, ...) are not clickable to jump to their definitions.
