document the shape of operations and snapshot

I might be missing it somewhere, but it doesn't seem like the shape of operations and snapshots is documented anywhere.

@geakstr I think that's only for the operations of your given type. But ShareDB seems to have its own concept of operations that wrap those but that aren't documented well.

Anyone have more information on this? Specifically...

• What is the op.seq property? and is it required to be saved? or can it be inferred somehow?
• Does op.src need to be saved? or is that just a temporary thing since it's the ID of the client?

I was planning on just saving op.op and op.v to the database, but unsure if that is sound.
