Also fix drawing the 3D mesh. Add the coordinate in the third dimension.
Note: The metric value is not verified, but the mesh looks OK in the viewer.

Add a single tetrahedron to show that the 3D mesh code basically works, although may have some issues that are not known.
This is to address issue #60.
