Pls update information on token adding

Outdated info in readme

To Reproduce
check this:

1. Add link to sushiswap/default-token-list Issue - the repository is read-only now, nobody can add issues. What shall users do?
2. Run npm run check and make sure tests pass - can't even install dependencies, it fails (mac os, can give more info, but I think it can be easily reproduced)

Expected behavior
Valid steps to add a token in readme
